I think it's the connotations of underwear that makes it different - typically you don't see somebody's underwear unless you're getting frisky,  and if you see it otherwise it's still something private and personal being exposed. It's not designed to be seen and its 'usage' reflects that, and where bikinis have been designed for the public eye, their usage reflects that.
We're conditioned to think that bikinis are acceptable outer-wear nearby a pool or at a beach because it's what we experience from early on. Most bikinis have traits that set them apart from underwear, including material, design, colour and, well, overall look. Subtle though the differences are I think you'd notice immediately if someone was strolling around on the beach in their underwear.
