How often do you take a small walk/jog/run around the block?

I run at least once per day.

How often do you watch TV?

During the summer I used to watch it for 1~3 hours of Grey's Anatomy Mon.-Fri. (depending on tasks to be done) and 1 hour of House every Sunday. Now, never.

How often do you get scared?

You have never been scared? xD Hard to believe such a person exists.

Once, nearly drowned in neighbor's pool. Would like to learn though.

How often do you go to an amusement park?

Maybe four or five times a year?

How often do you eat chocolate?

Every day
