This annoys me. Looking cool is not the only reason people smoke. Nicotine has a very relaxing effect that helps calm and relieve stress. OP even said he had to sneak out and go hide somewhere to smoke, and it'd be pretty pointless to try 'look cool' where nobody can see you...
I don't smoke and I think it's nasty, but that's my personal preference. Everyone needs to stop being so judgmental and screaming about cancer every time smoking is brought up. Cancer results from smoking heavily for decades, and OP is just a teenager with a long life ahead of him still.

If you have the opinion that smoking gives cancer after many decades then you can just put it off for years thinking your fine. Also how do you think he started smoking? My guess would be with friends, hence the looking cool statement. To me the risk of being killed from something is over the calming and stress relieving effect of it.

Well, there's a chance that she didn't see anything at all.
Also, there's a chance that she interpreted it as cold breath maybe?
But, if things turn out the other way round, what my friends would do in this situation is deny it.
At the end of the day, she's seen you just once, not many times to prove her point. No evidence too. So, it could all be a misunderstanding. I don't know if it will work in your case, but it seems to have with some people. Best of luck.
